Man dies in forklift accident
August 1st 2010

A man, Andrew William Davies, 42 of Albury, has died in the West Midlands after becoming trapped between a forklift truck and a reversing lorry.

The incident occurred on the 1st July 2010 in the rear yard of Halesowen-based Mapei UK, an adhesive supplier to the building industry.

Black Country Coroner Robin Balmain said: “The medical cause of death appears to have been a fatal blunt force head injury. It seems he was in collision with a reversing lorry which then impaled him on the forks of his forklift.”

The HSE is investigating the incident.
