Man dies in tragic forklift accident July 13th 2010 A man has died in the West Midlands after becoming trapped between a forklift truck and a reversing lorry.
The incident occurred on the 1st July 2010 in the rear yard of Mapei UK an adhesive supplier to the building industry, situated in Coombswood Industrial estate in Halesowen, West Midlands.
The man later named as Andrew William Davies age 42 of Albury, was impaled on the forks of the forklift truck as the lorry reversed onto him.
An inquest into the incident was opened on the 6th July by Black Country Coroner Robin Balmain.
He said: “The medical cause of death appears to have been a fatal blunt force head injury. It seems he was in collision with a reversing lorry which then impaled him on the forks of his forklift.”
